Thank you for your interest in starting your journey with Adoption Center of Illinois. The first step is to complete this Application. The information provided here helps our team determine the services your family needs. Your information will be held in the strictest confidence. Make sure you've blocked about one hour to complete this form. Some requests may require you to save and come back to the form once you have the requested information. Should you need to, just re-sign in through the link. 

Please note that your application will not be processed until the Phase I Fee has been received. Instructions for payment can be found at the end of the application. Before applying, please review our fee schedule HERE!

Our agency approves Illinois families for adoption according to DCFS standards and Illinois law. Our programs are open to people regardless of race, ethnicity, gender, gender identity, gender expression, sexual orientation, age, religion, mental or physical disability, marital status, length of marriage, size or composition of existing family, participation in infertility treatment, or any other factors protected by law. Applicants must be at least 21 years of age.
